Police allege the Volkswagen Tiguan was being driven erratically along the Hume Fwy in Euroa just before 11am.
Police allege the vehicle was stolen from an address in Werribee on May 28.
Later on Thursday, the police Air Wing observed the vehicle travelling on the Northern Hwy at Bylands, near Kilmore, where the driver allegedly lost control and crashed into a barrier.
The driver was arrested and taken to hospital under police guard.
The Werribee man, 32, has since been interviewed and charged with several offences, including reckless conduct endangering serious injury, dangerous driving while pursued by police, three counts of failing to stop on police direction, and driving in a dangerous manner.
He was also charged with three counts of unlicensed driving, two counts of theft, criminal damage, unlawful assault, theft of a vehicle, failing to answer bail, and three counts of committing an indictable offence on bail.
The man will appear before Broadmeadows Magistrates’ Court.
